New Patient Registration

If you live within our Practice area, you are welcome to register with us. Our reception staff will be happy to guide you through the process. 

We would ask that you send copies of the following documents to the Practice:

  1. BSO Application (see links below depending on whether you are transferring within N. Ireland / U.K. or coming from outside the country) or medical card

    • BSO Application (HS200 2021) for those transferring within N. Ireland and U.K.

    • BSO Application (HSC R1 2021) for those coming from outside N. Ireland and U.K.

  2. New patient application form (link to PDF)

  3. Photographic ID

  4. Utility bill with current address

  5. Summary printout from your previous GP 

Medical treatment is available from the date of registration.

New baby registration

Please ensure you register a new baby as soon as possible to allow information to be received from hospital and appointments to be scheduled for baby clinic.

This can be done by completing a BSO Application or submitting an HS123 - Infant Registration Form issued by the Registrar of Births when a birth is registered

Temporary residents

If you are ill while away from home, or if you are not registered with a doctor, you can receive emergency treatment from the local GP practice for 14 days.
